The clinic

Centrum Stomatologii Dentalblue Wawrzyn is at Kazimierza Pułaskiego 35 in Białystok. Its published terms describe it as a medical entity operating under a permit from the Podlaskie Voivodeship Office. The clinic provides private care and services funded by the NFZ.

Registration

Register in person or by telephone: 85 748 00 50, 787 906 376 or 698 071 007. Currently available booking methods are listed on the Contact page. Registration operates from 8:00 to 20:00.

Appointments are booked in order of enquiry. Based on the patient's information, reception selects a specialist and visit length; for NFZ services it provides the first available appointment.

A standard appointment lasts 45 minutes and an endodontic appointment lasts 1.5 to 2 hours. Reception confirms the appointment by telephone or text the day before. The dentist consults in the surgery, at the chair, not by telephone.

The first appointment

Arrive 15 minutes early to complete records and have a panoramic X-ray. The published clinic terms quote 120 zł for that image. You may bring your own panoramic X-ray if it is no more than one year old.

The patient presents an identity document confirming their personal details. An individual medical record is opened and a health questionnaire completed before the appointment.

Health questionnaire

Complete the questionnaire at the first appointment and update it at subsequent appointments if your health changes. Under the published clinic terms, refusal to complete it results in the dentist declining to provide the service.

Medical records

Records are kept digitally. The scope of data follows the Act of 6 November 2008 on Patients' Rights and the Patients' Rights Ombudsman: at least name, date of birth, sex, address, PESEL number, provider identification, health status and the date of preparation.

The clinic keeps, stores and releases records in accordance with the law and with confidentiality protected. The maximum release fee is set by article 28(4) of that Act.

Confirming and cancelling appointments

If the patient does not confirm by telephone or text by 16:00 on the day before the appointment, it is cancelled and the patient is notified by text.

If attendance is not possible, cancel or reschedule promptly. Arriving more than 15 minutes late is treated as cancellation; the next patient is then admitted.

After two instances of failing to attend a confirmed appointment or cancelling on the appointment day, full prepayment is required to book again.

Payment in advance for longer appointments

For visits longer than 45 minutes, the published terms require a 400 zł deposit, described as zadatek, which is deducted after treatment.

The payment is forfeited for nonattendance or cancellation less than seven days before the appointment. The terms also provide for no refund if the appointment is not confirmed; an L4 medical certificate is the stated exception.

Warranty

The clinic provides a warranty for completed work on the conditions below. Its period runs from completion of the work. Concerns about a procedure can be discussed with your dentist.

• Regular checkups at the dental surgery.
• Professional hygiene, including scaling, air polishing and fluoride treatment, every six months.
• Examination based on a current panoramic X-ray no more than 12 months old.
• Following the dentist's recommendations on treatment and oral hygiene.

Warranty exclusions

• Temporary work, including temporary crowns and immediate dentures requiring relining.
• Work performed at the patient's express request after they have been told that the warranty is limited or excluded.
• The need for repeat endodontic treatment.
• Replacing a dressing alone instead of the recommended root canal treatment.
• Repairs by the patient or third parties, and repairs outside Dentalblue, except warranty repairs.
• The aesthetic effect of whitening, which depends on factors including enamel, discolouration, previous treatment and lifestyle. The published terms say that treatment sometimes needs repeating after about two weeks.
• Prosthetic or aesthetic work, including bonding, without prior professional hygiene treatment.
• Prosthetic or aesthetic work before comprehensive treatment of every tooth requiring care, including decay, inflammation and root canal treatment.

Payment

Payment is due after each appointment according to the applicable price list.
