Privacy policy
How this website handles data.
Updated: 8 September 2026.
Who is responsible for the data
The data controller is Dentalblue Wawrzyn, ul. Kazimierza Pułaskiego 35, 15-337 Białystok. NIP 5423458246, REGON 523549592.
Data enquiries: dentalblue@dentalblue.pl, telephone 85 748 00 50.
The clinic is a medical entity operating under a permit issued by the Podlaskie Voivodeship Office in Białystok. It is a private practice and also provides treatment under the NFZ.
What this site does not do
This website runs no analytics, profiling or advertising pixels of its own. The optional Google map uses an external provider, as described below.
This website has no contact form. The online booking link, where available, opens the external Estomed service, whose provider sets out its own data protection terms.
Cookies
Your consent decision is stored locally in your browser to remember your choice on later visits. You can change it through Cookie settings in the footer or delete the stored entry through your browser settings.
The optional Google map may use cookies and similar technologies after you grant maps consent.
The Google map
The map on the Contact page loads after you grant maps consent using Show map, the banner acceptance button or Cookie settings. If consent is already saved, the map loads when you return to Contact. Without that consent, the embedded map sends no requests to Google.
When the map loads, Google receives technical data, including your IP address and browser information. You can withdraw consent through Cookie settings in the footer; this removes the embedded map. Withdrawal does not reverse data already sent.
Opening an external Google Maps link takes you directly to Google’s service.
Telephone calls
Contact details given on a phone call are used only to arrange an appointment.
Who we pass data to
The clinic's published policy names these groups of recipients:
- the hosting company, under a processing agreement
- authorised employees and associates
- companies providing marketing services to the controller
Transfers outside the European Union
After you grant maps consent, Google may process data on servers outside the European Union. Google describes its transfer safeguards in its privacy policy: https://policies.google.com/privacy and https://policies.google.com/privacy/frameworks.
How long we keep data
Data is processed no longer than is necessary to carry out the activities set out in separate regulations, for example accounting rules.
Medical records are generally retained for 20 years, counted from the end of the calendar year of the last entry. Certain categories have different retention periods under article 29 of the Act on Patients’ Rights and the Patients’ Rights Ombudsman.
Medical records
Medical records are kept digitally. Their scope is set by the Act of 6 November 2008 on Patients' Rights and the Patients' Rights Ombudsman.
The records contain at least: name and surname, date of birth, sex, home address, PESEL number, the identity of the provider, a description of the state of health, and the date of preparation.
The maximum fee for releasing medical records is set by article 28(4) of that Act.
